Presumably some of the LH prop blades broke off & cut through the fuselage & caused the fatality. The RH engine seemed to run on for quite some time.

That CVR reveals some pretty sloppy airmanship in the cockpit, and a poor working relationship between crew members - all culminating in a disastrous landing.
